1. Please complete this form if you purchased or received a ticket for the November 16, 2023 F1 Las Vegas FP1/FP2 event (other event dates do not qualify).
2. AN ATTORNEY CLIENT RELATIONSHIP CANNOT BE ESTABLISHED UNTIL AFTER A CLASS IS CERTIFIED BY THE COURT. We are requesting the court to certify an “opt out” class, which means everyone who qualifies for compensation is automatically included unless they opt out. If the court does not certify an opt out class, then we will contact those who submitted this form so that they may opt in.
3. Please be patient we have over 35,000 potential claimants and the litigation process moves slow.
